IT IS a practice fraught with emotion, legal complications and agonizing decisions for all involved. Because there are neither clear-cut rules nor clear-cut ethical guidelines, these life-and-death decisions often hinge on individual ideas about what constitutes a life worth living. say they privately worry about making mistakes, nurses describe their agony over whether they can emotionally bear to take care of a child left to die, hospital lawyers say they are nervous about liability, and parents are sometimes paralyzed by grief -or bitterly angry. "The whole process is terrible," said one intensive-care nurse who finally "burned out" and left the field.

"We're talking about people who, in the traditions of our culture and cultures throughout the world, have been left alone by natural law," said Duff, professor of pediatrics at Yale-New Haven Hospital, on a recent television show. "The argument been that what nature or God has created extremely imperfectly should be left alone and go back to nature." But now thousands of babies each year who previously would have died can be saved by surgery -especially children afflicted with spina bifida (exposure of the spinal column) and Down's Syndrome. About 5,000 children are born each year in the United States with Down's Syndrome. "These children are affected by attitudes about mental retardation," said Diamond. "No one knows at birth how severely retarded a child will be. If (treatment isn't performed), it's because the parents or doctors don't think a mentally retarded life is worth living." The recent death of a baby born with Down's Syndrome in Bloomington, was the catalyst for the government's warning to hospitals -a warning that will only intensify the confusion, say hospital administrators. "DOCTORS AND parents make these life and-death decisions, not hospital administrators," said Michael Bromberg, a spokesman for the Federation of American Hospitals.
